To become a member, or renew your membership, please complete the forms below and send them to phx.hispanic.network@phoenix.gov.

A few items to note:

Department Head signature is not required on the EDF Form. The HR Director has been authorized to sign on behalf of department directors.
Payments using Management Development Funds will be paid using a Payment Control Document. Only the PHN Application will need to be sent to PHN.
